I get this every time the conditions below exist, but its difficult to screen shot as it depends on a semi-radom event (an AI flipping). However should be easy enough to setup to verify in the Lab environment.
Conditions: When the end turn button is pressed, and there is a list of 'GNN Breaking News' ready to come on screen, and an AI(s) has a planet(s) ready to flip at turn end.
Reproduce By :
1. Press the end turn button
2. When GNN breaking news appears, select any of your own planets in the list and 'go to' the planet using the go to button on GNN news.
3. Click on any tile as if to change/amend the tile contents
As you do step three, the Planet thats flipping overlays the screen and you get a merged image of the originally selected planet, and the flipped planet. A quick workround is to press any resource focus button at the top of the planet screen which forces a screen-redraw, and you end up with the flipped planet front of screen with focus. Simulate doing any changes wanted to the flipped planet tiles, close and return to GNN news.
If there are more than one flipped planet queued up, it can now get really messy, as when you select close, the second flipped planet comes on screen and merges again. If there are several planets flipped, it is almost impossible to resolve (they all appear one at a time), and you end up noting the name of the planet(s) involved for manual checking after the GNN screen is closed.
Sometimes I make a couple of changes to the flipped planet, and before I can finish the changes, along comes the next planet in the queue of flipped planets, merging with whatever is on screen. That one is really confusing as you dont expect it, and at times forget what you were changing / wanting to change.
Difficult to screen shot as it depends on a planet having flipped after GNN news appears, that is difficult for us to guarantee so that a screen shot is able to be obtained.
Hopefully thats not as clear as mud

Its difficult to describe when there is no chance of a screenshot ....
