I think I have been observing this for some time now, not sure just when I first noticed it, but just hadn't been able to pin down what was causing it. Now I know. My survey ship just explored a worm hole anomaly and must have left a sensor behind.
I think this is just a case of the FOW not being restored after the ship makes the jump.
