I had this same problem the other day. Turned out to be the Realtek HD Audio driver just disappeared. Something is definately up with that driver on the Windows side of my setup and is causing the issues. I'm running an iMac in Boot Camp, but I think that inconsequential to the issue with...as I have crystal clear audio on the OS X side, and choppy audio on the Vista side.
